Becoming a Client

To access Laura Johnson Home Residential Care Services a number of documents are required.

Here is a Checklist of Documents for entry to Residential Care:

  • Aged Care Client Record
  • Application for Residential Aged Care
  • Letter received from Department of Social Services outlining Residential fees and charges
  • Medication Summary from your GP
  • Pension and Medicare Card

Once you have gathered all required documentation please contact the Laura Johnson Home to organise an initial meeting to discuss residential care options.

Success of applications for residential care can vary according to room availability and resident care needs.

Documents & Links

Applicants will be required to undertake assessment by an Aged Care Assessment Team (ACAT). This can be organised by contacting your local ACAT team directly. The Aged Care Assessment Team will give you an Aged Care Client Record which will need to be bought with you to initial application discussions.

Applicants will need to complete an application for residential aged care.

Completion of the Permanent Residential Aged Care Request for a Combined Assets and Income Assessment form is required to determine potential fees and charges. Please note the wait time for return of this assessment can take a number of weeks so it is important this is completed as soon as possible.

Residents should make an appointment with their GP to obtain a medication summary which lists your medical history and current medications. It is important this list has been updated with all recent changes to medications.   This summary is a vital part of the application discussion.
